Established Patients 

We will call you two weeks prior to your appointment to remind you of the following:

  • Appointment date and time

  • Bring all your prescription, over-the-counter and herbal medications, including vitamin supplements, (preferably in original containers)

  • Please have your labs drawn and/or diagnostic tests completed one week prior to your appointment.  Our on site lab is open Monday through Thursday.  Please call for an appointment. 

You will also receive an email reminder if you have web enabled access to the patient portal.

 

IMPORTANT REMINDERS:

It is important to have your labs drawn one week prior to your appointment due to the varying processing times required for different lab tests. This will allow time for all tests to be processed and for us to receive the test results for your physician to review at the time of your appointment. Your lab results provide crucial information about how your kidneys are functioning and must be provided in order for your physician to monitor and treat your condition effectively. If we do not have your lab results, we may need to call you and ask to cancel your appointment. We will reschedule your appointment to allow you additional time to have your labs completed.

We ask that all lab and diagnostic tests be completed Monday through Thursday during the week. This allows time for test results to be reviewed during office hours, and time to contact you in case of abnormal results before closing the office for the weekend.

Remember that if your insurance plan requires a referral from your primary care provider, you must contact the provider prior to your visit. We are happy to assist you in obtaining your referral; however, please be aware that it is the patient’s ultimate responsibility to obtain the proper referral for care.

Please be prepared to pay your co-pay based on your insurance at the time of service. We accept cash, personal checks, MasterCard, VISA, Discover, and debit cards. If you have any questions or concerns, contact our billing office staff at (405) 737-4900.
