New Patients

Please consult your primary care provider (PCP) before scheduling an appointment with us. Your PCP may call us to schedule your appointment or you can call us directly at (405) 737-4900.

Kellie Jackson is our New Patient Coordinator.  If you already are scheduled for an appointment, she will call you two weeks prior to your appointment to remind you of the appointment date and time.  We will ask you to bring all your medications and supplements (preferably in the original containers) and to have your labs drawn before your visit, if needed.  You will also receive an email reminder if you have web enabled access to the patient portal. 

If you have been referred from another physician, Kellie will call you to review your referral information and schedule an appointment with our physicians.  You can reach Kellie at (405) 737-4900, ext. 217.

Please arrive 15 minutes prior to your scheduled appointment time to complete the registration process, which includes taking a photo for your electronic medical record.Initial appointments vary in complexity and may require you to be in the office for 30-45 minutes.  Please plan your schedule accordingly.For your convenience, please complete and  bring your Demographic Information Form and Advanced Directive Form.  

Bring all your prescription, over-the-counter and herbal medications (including vitamin supplements) to your appointment.  We prefer that you bring the original containers for your medications. If you do not have the original containers, please bring a complete list of all medications, including the spelling as prescribed, strength, and dosage. Remember to bring your insurance card and photo ID.

Verify with your insurance company that Midwest Nephrology Group is covered on your plan. If you are part of an HMO or other policy that requires a referral to our office, please coordinate with your PCP to make sure a referral has been sent prior to your appointment.

Be prepared to pay your co-pay, based on your insurance at the time of service. We accept cash, personal checks, MasterCard, VISA, Discover, and debit cards.  If you have any questions or concerns, contact our billing office staff at (405) 737-4900.
