Where is your office located? Midwest Nephrology Group, PLLC has four convenient locations to serve you in Midwest City, Oklahoma City and Shawnee. Our main office is located in Midwest City, OK at 9230 E. Reno Ave., Suite B.
We have two offices in Oklahoma City, located in the Oklahoma Heart Hospital south campus at 5200 I-240 Service Road, Suite 304 and in the Integris Deaconess Medical Specialist Building, 5701 N. What are your office hours? Our main clinic (Midwest City) is open from 8:00 A.M. to 5:00 P.M. Monday through Friday. Please call (405) 737-4900 for clinic hours and availability at our other clinic sites.
How do I make an appointment as a new patient? We accept referrals from other physicians only. Please contact your primary care physician if you would like to schedule an appointment.
What do I need to bring to my first appointment? Photo identification card, insurance card(s), and completed Demographic Information Form and Advance Directive Form. Bring all your prescription, over-the-counter and herbal mediations, including vitamin supplements, (preferably in original containers). If you do not have the original containers, bring a complete list of medications that provides the name (spelled exactly as prescribed), strength and dosage.
Do your physicians see patients in the hospital? Yes, we make daily rounds at local hospitals.
How can my prescriptions be refilled? Request refills from your pharmacy. It is our office policy not to call in/refill prescriptions for sleeping pills or pain medications on nights or weekends. These refills need to be requested during regular office hours. Please allow us 48 business hours to respond and/or refill your prescriptions.
How do I find out if you accept my insurance? Call your insurance provider.
What do I do if my insurance has changed? Call our office at (405) 737-4900 and select option #4 to speak with the Billing Department. Please bring new insurance card(s) to your next appointment so we can make copies for your file.
What if I get sick after hours or on the weekends? Call our office (405) 737-4900 and the answering service will direct your call.
What happens if I cannot keep a scheduled appointment? Please call (405) 737-4900 at least 24 hours in advance so that we can accommodate other patients during that time and give you the next available appointment.
How can I get my laboratory work completed? We ask that you schedule your appointments for lab work one week prior to your scheduled office visit. We have a lab at our main office location. Please call (405) 737-4900 to make an appointment, Monday-Thursday. If you prefer another lab, let us know which laboratory you prefer to use and we will send your lab orders to that location.
